3. Client Bank Information.

Note: A voided check from the debited account must be attached with final enrollment.

4. Debit Options
$*to be debited every two weeks, which is 1/2 of the normal monthly mortgage
                   payment. This amount will be forwarded to my mortgage company. This
   *(or)         amount also includes any additional principal pmts. (if any) of $

$*to be debited twice monthly, on the fifth and the twentieth days of each
                 month. If debit transfer falls on a weekend, transfer shall occur on the Friday
                  before. This amount includes 1/2 of the normal monthly pymt. plus 1/24 of
                  pymt. amt.

                (depending on option chosen above), which is a service fee to cover processing.

    Check only if applicable. I / we would prefer to be billed directly once a month
           for our current mortgage payment plus 1/12 that amount instead of the bi-weekly
            bank debit. There is a $7.50 mo. handling fee. All benefits of the program are
            unchanged.

This includes a $5.95 service fee per debit for twelve (12) debits This plus the initial enrollment down payment of will be our total enrollment fee. Upon our cancellation of the Mortgage Savings Program tm, any unpaid balance of the Enrollment Fee may be offset against the Account.
* Continue to make monthly payments to lender until notified to stop ( usually 1mo.)

    business days of final enrollment.
